I agree to inform NLHP immediately of any state or federal investigations, criminal charges or convictions, particularly with regard to federal healthcare or procurement programs.

The extension of all job offers may be conditioned upon successful completion of a post-offer physical examination and/or drug screen. Failure to submit to, complete or pass a drug screen will conclude all consideration for employment for one (1) year.

In compliance with the Federal Immigration and Reform Act of 1986, all individuals hired must complete an Employment Eligibility Form (I-9) and verify identity and employment authorization prior to employment.

I agree to conform to the policies and procedures of NLHP and understand that NLHP or its employees have a right to freely enter into the employment relationship and sever this relationship at any time for any reason. I understand that no management representative of NLHP has any authority to enter into any contract for employment.

I understand that if my application is deemed incomplete by NLHP, it will not be considered further.

I certify that the information in this application is complete and correct to the best of my knowledge. I understand that NLHP shall Not be liable in any respect if my employment is terminated because of false statements, incomplete answers, or omissions made by me on this application.
